IEHP DualChoice members with chronic conditions who qualify for the Vibrant Health Utilities Benefit, get $65 per month via the Vibrant Health Debit Card to help pay for utilities such as gas, electricity, sanitation and water.
IEHP will decide if you qualify and inform you.
To qualify, you must meet the following criteria:
1. You are diagnosed by a plan provider with one or more of the following chronic condition(s) identified below and meet certain medical criteria, you may be eligible for a utilities allowance, a targeted supplement benefit:
- Autoimmune Disorders,
- Cancer,
- Cardiovascular Disorders,
- Chronic Alcohol and Other Drug Dependence,
- Chronic and Disabling Mental Health Conditions,
- Chronic Heart Failure,
- Chronic Lung Disorders,
- Dementia,
- Diabetes,
- End-Stage Liver Disease,
- End-Stage Renal Disease,
- Severe Hematologic Disorders,
- HIV/AIDS,
- Neurologic Disorders,
- Stroke
2. In addition to having at least one or more of the chronic conditions listed, you must have high risk health status as determined by IEHP.
Your monthly allowance is available on the first day of each month. Unused amounts roll over from month to month. You must use all funds by the end of calendar year.
Expenses can only be used for utilities received within San Bernardino and Riverside counties.
For the Utilities benefit, you must have at least one chronic condition such as (cardiovascular disorders, chronic and disabling mental health conditions, chronic lung disorders, diabetes mellitus, and neurologic disorders) and meet other criteria to qualify. There may be other eligible chronic conditions that are not listed. For information on additional eligible chronic conditions see your Member Handbook. Eligibility for this benefit cannot be guaranteed based solely on your condition. All applicable eligibility requirements must be met before the benefit is provided. Please see your Member Handbook for more information. Medicare approved IEHP DualChoice to provide this benefit as part of the Value Based Insurance Design program. This program lets Medicare try new ways to improve Medicare Advantage plans.
IEHP DualChoice (HMP D-SNP) is an HMO Plan with a Medicare contract. Enrollment in IEHP DualChoice (HMO D-SNP) depends on contract renewal.
