Complex Care Management

Get the care you need

When you have a serious illness, it can be hard taking care of yourself, handling your meds and setting up doctor’s visits at the same time. At IEHP, we’re here to help make things simpler.

 

Our Complex Care Management (CCM) program helps members with two or more complex behavioral health or medical conditions that are not being well managed. It can help those having trouble managing:

 

  • Heart disease
  • Lung disease
  • Kidney disease
  • AIDS
  • Hepatitis C
  • Spinal injury
  • Mental health
  • Addiction (substance use, gambling, etc.)
  • And more

How we can help

Our Interdisciplinary Care Team (ICT) will work with you and your doctor to make sure you get the care you need. We can help you:

 

  • Support your health needs
  • Make a custom care plan
  • Address your mental health
  • Get needed resources and social services
  • Learn about your benefits and how to use them
  • Work with providers to get needed medical equipment

 

With our support, you’ll learn how to better manage your care over time.

How to opt into the CCM program

While a provider can refer you to the CCM program, you can also opt into the program yourself.

 

To get started, call IEHP Member Services at 1-800-440-IEHP (4347), Monday-Friday, 7 a.m.-7 p.m. and Saturday-Sunday, 8 a.m.-5 p.m. TTY users should call 1-800-718-IEHP (4347)

 

1. You must complete a health survey to assess your needs.

2. If you are eligible and wish to opt in, you will be enrolled.

3. Once enrolled, you will be assigned a primary care manager, who will:

  • Contact you at least once every 30 days to discuss your health goals.
  • Help you with a plan to meet your goals.
